SURG.00084 Implantable Middle Ear Hearing Aids

This policy addresses semi-implantable and fully implantable middle ear hearing aids. Anthem considers these devices investigational and not medically necessary; they are not covered for any indication (claims may be denied under codes 69799, S2230, V5095). There are no covered exceptions or clinical criteria—noncoverage applies regardless of hearing loss type, laterality, or severity.
