This policy addresses transmyocardial/perventricular device-based closure of ventricular septal defects (VSDs), including congenital or post–myocardial infarction VSDs. Anthem considers this approach investigational and not medically necessary; it is not covered for any indication. Claims may be submitted under CPT/HCPCS 33999 but will be denied as investigational.
