This policy addresses implantable left atrial hemodynamic monitoring devices used in heart failure management, such as the HeartPOD System, Promote LAP System, and V-LAP System. Anthem considers these devices investigational and not medically necessary; therefore, they are not covered for any indication. There are no covered indications or exceptions—use of these implantable monitors is excluded from coverage in all scenarios.
