SURG.00160 Implanted Port Delivery Systems to Treat Ocular Disease

This policy addresses implanted ocular port delivery systems (for example, the Susvimo implant) that are surgically placed and periodically refilled to continuously deliver medication into the vitreous for conditions such as neovascular (wet) AMD. Anthem considers port delivery systems investigational and not medically necessary for all indications; therefore, implantation, refills/maintenance, and related services are not covered. There are no covered exceptions or conditions—coverage is not provided for any indication.
