Cigna considers multiple ablative procedures, implantable/electrical neurostimulation (including generators/leads) and specified cranial nerve blocks (e.g., CPTs 64400, 64405, 64450, 64505; ablative codes 64553/64555/64568/64575/64590/64744/64999; HCPCS C1820/C1822/L8679–L8688) not medically necessary/not covered for headache (including chronic migraine, cluster, cervicogenic), occipital neuralgia, and trigeminal neuralgia and lists no covered services for these indications. Providers must bill with the appropriate current diagnosis and procedure codes or claims will be denied (reimbursement is not allowed for diagnoses/services excluded by this policy), and the policy notes no FDA‑cleared occipital nerve stimulation devices for these indications.
