Ultrasound guidance (CPT 76942) for trigger point injections is explicitly not covered or reimbursable and claims using 76942 or lacking covered codes for these injections will be denied. Providers must submit the most appropriate, currently effective CPT and diagnosis codes (deleted/not-yet-effective codes ineligible), and reviewers should consult the applicable benefit plan and current Medicare LCDs and apply medical director judgment.
