Cigna covers corneal relaxing incisions (CPT 65772) only to correct surgically‑induced astigmatism ≥3.00 D after cataract or corneal transplant in patients intolerant of glasses/contact lenses, and covers intrastromal corneal ring segments (CPT 65785) only for keratoconus meeting all FDA HDE criteria (progressive deterioration, age ≥21, clear central cornea, ≥450 microns thickness at the proposed incision site, and corneal transplantation as the only remaining option). All other corneal refractive procedures are deemed not medically necessary or investigational, and claims require appropriate CPT/diagnosis codes and clinical documentation showing the policy criteria are met and are subject to the member’s benefit plan.
