Cigna covers chromoendoscopy (dye-based or virtual/electronic techniques such as NBI) only for surveillance of Barrett's esophagus and for colorectal cancer surveillance in patients with a personal history of inflammatory bowel disease, and excludes other indications. Coverage requires appropriate and current procedure and diagnosis codes (policy cites unlisted CPTs 44799, 45399, 45999 as considered), documentation of the surveillance indication, operative/endoscopy report documenting use of chromoendoscopy and any optical technology, and biopsy/pathology documentation as applicable; claims lacking covered codes or required documentation will be denied.
"Chromoendoscopy, with or without optical technologies such as Narrow Band Imaging™, is considered medically necessary for surveillance of individuals with Barrett's esophagus."
