Documentation must show pregnancy status (for antenatal cerclage) and at least one qualifying criterion, use the appropriate covered procedure codes (e.g., 57700, 59325 or applicable unlisted codes), and note that transabdominal cerclage is reserved after failed or anatomically difficult transvaginal attempts and cerclage is generally limited to the second trimester; IVIG/genetic testing coverage is determined by separate policies.
