Cigna covers antenatal transabdominal/laparoscopic cervical cerclage and cerclage on non‑pregnant individuals only for patients with prior failed history‑indicated transvaginal cerclage, prior cervical trauma (e.g., cone biopsy or laceration), or shortened cervical length, and considers multiple diagnostics and immunotherapies (e.g., inhibin B, placental PPAR/TNFα testing, NK cell testing, paternal/third‑party leukocyte immunization) and associated CPTs (e.g., 83520, 86357, 86950, 96365) not medically necessary or not reimbursable. Documentation must show pregnancy status (for antenatal cerclage) and at least one qualifying criterion, use the appropriate covered procedure codes (e.g., 57700, 59325 or applicable unlisted codes), and note that transabdominal cerclage is reserved after failed or anatomically difficult transvaginal attempts and cerclage is generally limited to the second trimester; IVIG/genetic testing coverage is determined by separate policies.
