Cigna covers non‑obstetrical transvaginal ultrasound only for evaluation of suspected pelvic pathology and for screening/surveillance in women at increased risk for ovarian or endometrial cancer (and for specific indications such as postmenopausal bleeding, pelvic mass, acute pelvic pain; infertility imaging is handled under a separate policy), and explicitly excludes TVUS for general‑population cancer screening and numerous listed diagnosis/CPT codes (including CPT 76830). Claims require a physician order plus appropriate covered diagnosis/procedure codes and supporting clinical documentation (e.g., symptoms or evidence of high‑risk status) or they will be denied.
