Cigna covers continuous intraoperative neurophysiologic monitoring (IOM) only for procedures with significant risk to the brain, spinal cord, cranial nerves, or major peripheral nerves (specific high‑risk spine, cranial, vascular, plexus and tumor surgeries listed) and excludes IOM for surgeries below spinal level L1–L2, routine lumbar/cervical decompressions in the absence of listed high‑risk conditions, epidural/SI injections, radiofrequency ablation, and placement of spinal cord/DRG stimulators or intrathecal pumps. Key requirements: monitoring must be continuous and interpreted in real time by a licensed clinical neurophysiologist (not the operating surgeon/anesthesiologist) who is immediately available (on‑site or via real‑time remote means), performed by qualified personnel with pre‑operative baseline studies limited to once per operative session and complete documentation (baseline tracings, intraoperative logbook, personnel qualifications, and real‑time communication).
