Cigna covers serum 25(OH)D testing (and limited 1,25(OH)2D testing only for specific disorders) for non‑pregnant adults age 18–64 when there is a listed diagnosis associated with vitamin D deficiency, previously documented deficiency, or suspected toxicity; routine population screening, testing for other ages or pregnancy, and combining CPT 82306 and 82652 are excluded. Coverage requires a covered ICD‑10 code and supporting medical record documentation (prior labs for repeat tests, clinical evidence for toxicity), limits 25(OH)D to ≤2 tests per 12‑month rolling period except for documented hypoparathyroidism, CKD, or intestinal malabsorption, and mandates specific justification when ordering CPT 82652.
