Coverage requires multidisciplinary team agreement (≥2 specialists) that LITT is the best option after considering alternatives, documentation of prior treatments (epilepsy = failure of ≥2 AED regimens), imaging/lesion size, rationale that craniotomy is not suitable when applicable, MRI suitability, and procedure‑specific surgeon training and hospital privileges.
