Cigna covers MR‑guided LITT only for (1) medically‑refractory epilepsy with a well‑defined temporal lobe or hypothalamic focus, (2) symptomatic, recurrent primary or metastatic malignant brain neoplasms ≤30 cc, and (3) symptomatic radiation necrosis ≤30 cc; all other indications are not medically necessary. Coverage requires multidisciplinary team agreement (≥2 specialists) that LITT is the best option after considering alternatives, documentation of prior treatments (epilepsy = failure of ≥2 AED regimens), imaging/lesion size, rationale that craniotomy is not suitable when applicable, MRI suitability, and procedure‑specific surgeon training and hospital privileges.
