Cigna covers transcatheter catheter ablation in adults (≥18) for specified supraventricular and ventricular arrhythmias — including accessory‑pathway/AVRT (or preexcited AF with high‑risk EP findings), CTI‑dependent atrial flutter (including when induced during AF ablation), symptomatic AVNRT, symptomatic focal AT, junctional tachycardia when medical therapy is ineffective/contraindicated, PVCs with burden >15% or symptomatic with failed/intolerant/contraindicated medical therapy, and ventricular tachyarrhythmias when medical therapy fails — but excludes ablation for inappropriate sinus tachycardia and premature atrial contractions and considers cardio‑neuroablation and thoracoscopic epicardial ablation experimental/unproven. Coverage requires documentation of adult age and objective arrhythmia evidence (ECG/ambulatory monitoring/EP study/device data), documentation of prior medication trials or contraindications where applicable, and correct CPT/ICD coding; claims lacking required codes or documentation will be denied.
