Cigna covers allogeneic HSCT for specified marrow failure syndromes, listed inherited metabolic disorders, primary immunodeficiencies and for sickle cell/thalassemia at increased risk, and covers autologous HSCT only for systemic sclerosis and for relapsing‑remitting/secondary progressive multiple sclerosis with disease activity; HSCT is excluded as not medically necessary for most other autoimmune diseases, certain metabolic disorders, adult autologous HSCT for SCD/thalassemia (except for Zynteglo collection), polycythemia vera and type 2 diabetes. Key requirements include an appropriately HLA‑matched donor for allogeneic transplants, age and strict disease‑specific criteria (systemic sclerosis age 18–69, disease ≤5 years with specified pulmonary/renal findings; MS age 18–55, EDSS 2.0–6.0, failed ≥1 DMT and recent relapses/MRI activity), documented diagnosis/prior therapies, and standard pre‑transplant organ function and infection screening with listed contraindications.
