Policy Summary
Cigna considers implantable peripheral nerve stimulation (PNS) for acute or chronic pain not medically necessary and implantable peripheral nerve field stimulation (PNFS) experimental, investigational, and unproven for all indications (PNFS reporting—including CPT unlisted 64999 and related HCPCS—is treated as experimental). Claims must be submitted with the appropriate covered diagnosis and procedure codes (using current CPT/HCPCS) and meet the applicable Coverage Policy criteria or they will be denied; the member’s benefit plan governs coverage.
