Cigna covers ambulatory external cardiac monitors (48 hours–30 days), MCOT, and implantable loop recorders for specified indications—e.g., suspected significant brady/tachyarrhythmia with presyncope/syncope/palpitations, AF evaluation when results will change management (including pre/post‑ablation), cryptogenic stroke/TIA, post‑CTI ablation in high thromboembolic‑risk patients not on anticoagulation, hypertrophic cardiomyopathy, recovered AF‑induced cardiomyopathy, and AF in acute illness/surgery (MCOT only after nondiagnostic external monitoring; pediatric ILR allowed with pediatric criteria). Key requirements: adult ILR requires exclusion of non‑arrhythmic causes and a minimum 14 days of inconclusive noninvasive monitoring (with documentation) before implantation and replacements only if criteria persist and device is no longer under warranty; routine/unspecified monitoring, ST‑segment analysis, consumer self‑monitoring devices/software, and any indications not listed are not covered.
