Cigna covers fixed‑wing air ambulance when transport to the nearest appropriate facility is required and either the pickup is inaccessible by ground, great distances/obstacles make ground transport infeasible, or ground transport would impede timely care — this includes urgent, time‑critical transplant events and specified scene/interfacility indications (trauma, cardiac, neurologic, neonatal, obstetric, burns, etc.), while residence‑based transplant travel, transport solely for clinical trial participation, and non‑nearest facility transports (except as specified) are excluded. Coverage requires clinical documentation that transport was to the nearest capable facility and that one primary criterion is met, supporting records for the claimed indication, use of appropriate codes (e.g., HCPCS A0430/A0435), and is subject to the member’s benefit plan and coding/utilization rules.
