Category III Current Procedural Terminology (CPT®) Codes - (0558)

Cigna considers CPT Category III (T) codes experimental/investigational and generally not covered unless a specific Cigna Coverage Policy explicitly provides coverage for that particular Category III code (a number of codes are explicitly listed as not covered). When a Category III code is covered under a related policy, coverage is limited to the indications, coding, documentation and prior‑authorization requirements in that policy (providers must use the appropriate, effective codes at time of service or claims will be denied).
