Cigna covers cervical plexus block as medically necessary for anesthesia or perioperative/postoperative pain control for procedures of the neck, shoulder, or clavicle (e.g., carotid endarterectomy, thyroid/parathyroid surgery, shoulder surgery, ORIF of the clavicle) and deems it not medically necessary when those criteria aren’t met. Coverage requires proper documentation and use of appropriate diagnosis/procedure codes (claims must include covered codes; CPT 64999 may be used when criteria are met), compliance with the member’s benefit plan, and adherence to applicable Medicare LCD/NCDs—claims without covered codes will be denied.
