Cigna covers subperiosteal (CPT 21245/21246), endosteal (CPT 21248/21249) and transosteal (CPT 21244) dental implants only when plan benefits exist and ALL specified criteria are met, and excludes implants for tooth loss from non-cancer decay, periodontal disease, cosmetic reconstruction, or other non-listed indications. Key requirements: repair must be impossible, conventional bridgework/partials not feasible (no abutment teeth), indication must be congenital/developmental, accidental injury occurring after coverage within 12 months (chewing injuries excluded), or cancer-related loss/reconstruction, replacement must occur within 24 months while covered, and claims need supporting documentation and the listed CPT codes.
