Cigna covers professional fees for licensed/certified midwife services — including planned home birth and immediate medically necessary postpartum/newborn care — when the midwife acts within their state‑defined scope and the member’s benefit plan provides coverage (in‑ or out‑of‑network reimbursement applies per network status); excluded are services by providers acting outside their scope, doula services unless mandated, many home‑birth supplies/equipment and home facility charges, duplicative or non‑medically necessary services, and out‑of‑network care when out‑of‑network benefits don’t exist. Key requirements: proof of midwife licensure/certification and scope under applicable state law, member plan eligibility and network status, clinical documentation of delivery/immediate postpartum care, and compliance with state mandates/licensure and plan terms.
