Cigna covers elective and therapeutic abortions and medically necessary treatment of abortion complications (including ectopic pregnancy), subject to the member’s benefit plan terms and applicable state law. Key requirements: claims must use current CPT/HCPCS/ICD codes and include supporting clinical documentation distinguishing elective (e.g., Z33.2 ± Z32.01) versus therapeutic indications or complications, demonstrate underlying maternal/fetal conditions when required, bill drugs (S0190/S0191) separately from S0199, and comply with plan and state regulations.

"Elective abortion: termination of pregnancy at the discretion of the woman for reasons other than maternal health or fetal disease. Elective abortion is reflected when ICD-10-CM code Z33."
