Cigna covers chiropractic services when medically necessary for diagnosis and treatment of musculoskeletal and related disorders (e.g., spinal pain, TMJ, extremity joint dysfunction) and allows specified manipulative procedures as part of multimodal care (including UE manipulation for shoulder complaints and LE manipulation for ankle inversion sprains), but excludes maintenance/preventive care, non‑musculoskeletal or routine/fitness/educational services, many passive modalities, and techniques/devices deemed experimental (UE manipulation for epicondylitis/carpal tunnel is not considered medically necessary). Key requirements: an individualized, time‑framed plan with objective measurable functional goals and documented progressive improvement delivered by a qualified chiropractor, detailed initial evaluation, daily timed treatment records (max 1 hour/4 timed codes per day), periodic progress reports and reevaluations, and generally a short trial of care (typically 2–4 weeks) with limited ongoing care after maximum therapeutic benefit.
