Cigna covers supervised biofeedback by licensed clinicians only for a limited set of indications — adult dyssynergic chronic constipation; selected fecal incontinence and refractory levator ani syndrome with preserved rectal sensation and voluntary sphincter control; stress/urge/mixed/overflow urinary incontinence after failed conservative measures; migraine and tension‑type headaches as part of comprehensive care; and post‑stroke muscle re‑education — and explicitly excludes other indications, in‑home devices, EEG/neurofeedback, the InTandem system and specified device codes (E0746, E3200) as experimental/not medically necessary. Coverage requires a written treatment plan, documentation of failure/intolerance/contraindication to specified conservative therapies when applicable, appropriate physiologic testing/clinical evaluation (e.g., rectal sensation, anorectal manometry), one‑on‑one face‑to‑face administration by a licensed/qualified practitioner in a clinical setting, and correct CPT/ICD coding (with adult restriction for the constipation indication).
