Policy Summary
Cigna covers RFA (CPT 60660, 60661) for differentiated and medullary thyroid cancer, autonomously functioning nodules causing hyperthyroidism, and for benign nodules only when compressive symptoms are documented and treatment is limited to a single RFA per 12‑month period; RFA for other diagnoses is not covered. Claims must use current appropriate CPT codes and be accompanied by documentation supporting the covered indication (diagnosis or compressive symptoms), and claims with unsupported or invalid codes will be denied.
