Implantable cardioverter defibrillators (ICDs) are covered for patients with life-threatening ventricular arrhythmias or symptomatic heart failure with reduced ejection fraction, provided they have undergone optimal medical therapy for at least three months. Key requirements include a shared decision-making interaction for certain patients and specific documentation of the arrhythmia diagnosis and medical history. Limitations include non-coverage for non-life-threatening arrhythmias, individuals under 45 years old, and those deemed not candidates for defibrillation.
