Covers hospital beds and related support surfaces (fixed‑height, variable/semi/total electric, heavy/extra‑heavy duty, and air‑fluidized beds) for patients who need special body positioning or head‑of‑bed elevation greater than 30 degrees, assistance with transfers (e.g., spinal cord injury, stroke, severe arthritis, fractures, multiple limb amputees), morbid obesity (>350 lbs to ≤600 lbs or >600 lbs), or stage III/IV pressure ulcers (air‑fluidized beds). Coverage requires meeting fixed‑height bed criteria as a prerequisite, trial of pillows/wedges for head elevation and at least one month of conservative wound care when applicable, adherence to device‑specific weight and clinical restrictions, and is subject to the member’s benefit plan and other exclusions (e.g., head elevation ≤30°, caregiver services, architectural modifications).
