This policy covers the use of an intraurethral valve‑drainage device (e.g., inFlow) as an alternative to intermittent catheterization for management of permanent chronic urinary retention due to impaired detrusor contractility (detrusor underactivity). Coverage is limited to female patients age ≥18 with neurologic‑origin IDC who can operate the device or have a trained caregiver, requires physician initial sizing/insertion, device replacement no more than every 29 days, and continued coverage requires an in‑person re‑evaluation (day 31–91) documenting symptom improvement and practitioner verification of adherence; benefits remain subject to the member’s plan terms.
