Preface to the Imaging Guidelines

This policy covers eviCore’s evidence‑based prefatory guidance for medical imaging requests, outlining appropriate advanced and non‑advanced imaging for common symptoms and routine surveillance (e.g., lung, thyroid, liver/pancreas/renal lesions, high‑risk breast MRI, oncology surveillance, cardiac stress imaging, neurological conditions, renal stones) including pediatric patients (typically birth through 18–21 as clinically appropriate). Major limitations/requirements: atypical presentations or requests not specifically addressed require physician review, coverage is subject to Health Plan/CMS policies and investigational determinations, clinician judgment may override the guidelines, and many advanced imaging requests require recent clinical evaluation or documentation (generally within 60 days).
