This policy covers coverage criteria for cardiac rhythm implantable devices—including ICDs, CRT‑D/CRT‑P, permanent pacemakers and leadless devices—used to prevent or treat ventricular arrhythmias, bradyarrhythmias, and to provide cardiac resynchronization in heart failure. It applies to patients with structural heart disease, symptomatic heart failure (NYHA II–IV), documented ventricular tachyarrhythmias or congenital long QT syndrome, but requires prerequisites such as three months of maximally tolerated guideline‑directed medical therapy, and includes explicit non‑indications/exclusions, physician review for atypical presentations, and payer/Medicare‑specific limitations (including investigational‑device policies).
