Addendum to the eviCore Imaging Guidelines

This addendum covers chest CT imaging (CT chest without contrast CPT® 71250 and low‑dose CT chest CPT® 71271) for patients with contrast contraindication, follow‑up of pulmonary nodules, high‑resolution CT, annual low‑dose lung cancer screening per the NCD (and incidental nodules), and also addresses generator replacement (CPT® 33227/33228/33229/33262/33263/33264, 0614T) when device interrogation shows nearing ERI/EOL or documents malfunction and replacement is with a same or similar device. Major limitations: CPT® 71271 is restricted to annual screening only (interval follow‑up <1 year should be coded as CPT® 71250), MR spectroscopy for spine, CT breast techniques, and CMR strain/stress imaging (C9762/C9763) are considered investigational, and generator replacement outside documented ERI/EOL or malfunction or with dissimilar devices is not supported.
