This policy provides clinical guidelines for the use of imaging (diagnostic, staging, surveillance, and treatment‑response imaging) in oncology. It applies to a wide range of cancers (e.g., primary CNS, head and neck/salivary, skin/melanoma, thyroid, small cell and non‑small cell lung, breast, sarcomas, pancreatic, GI, colorectal, renal, transitional cell, prostate, etc.) and requires a recent clinical evaluation (within 60 days) and usually histologic confirmation of malignancy/stage, limits routine advanced imaging in asymptomatic individuals, restricts repeat studies absent clinical change or specified surveillance intervals, mandates physician review for atypical presentations, and does not support studies not listed in disease‑specific sections.
