This policy covers cardiac imaging services and the associated clinical decision support tool across modalities—including echocardiography, nuclear cardiac imaging, cardiac CT (CCTA and coronary calcium scoring), cardiac MRI, PET, and diagnostic heart catheterization—for evaluation of common cardiac symptoms and symptom complexes. It applies to patients with suspected ischemia/angina (including anginal equivalents), syncope, heart failure, cardiac trauma, arrhythmogenic cardiomyopathies and related indications, but requires a recent clinical evaluation (typically within 60 days), prior basic testing (ECG, chest x‑ray or echo, and prior stress test documentation), optimization of medical therapy when appropriate, and physician review for atypical or unaddressed presentations; imaging must be expected to affect management to be covered.
