Head Imaging Guidelines

This policy covers head imaging studies — including CT, MRI (brain, orbit/face/neck, IAC), CTA/MRA, PET and other neuroimaging — for evaluation and surveillance of common head-related symptoms and conditions (e.g., taste/smell disorders, ataxia, behavioral/neuropsychiatric issues, multiple sclerosis, tumors, hydrocephalus) and guideline-supported follow-up imaging. Major requirements and limitations: a pertinent clinical evaluation with detailed history and neurological exam is generally required before advanced imaging (exceptions exist for scheduled surveillance and certain conditions or when requests come from specified specialists or via documented meaningful contact), duplicate studies covering the same anatomic region with the same modality are generally not supported, and atypical presentations require physician review.
