This policy covers cardiac imaging procedures—including echocardiography, nuclear cardiac imaging, cardiac CT/CTA/CAC, cardiac MRI, cardiac PET, diagnostic heart catheterization, and pulmonary artery/vein imaging—for common symptoms and conditions such as congestive heart failure, cardiac trauma, adult congenital heart disease, cancer therapeutics–related cardiac dysfunction, and pre‑surgical cardiac testing. Major requirements include a current clinical evaluation and recent ECG within 60 days, completion of relevant non‑advanced testing (e.g., ECG, chest x‑ray, prior ECHO/stress tests) after symptom onset, and performance of advanced imaging only when it will change management; atypical or unaddressed presentations require individualized physician review.
