Pediatric Head Imaging Guidelines

This policy covers pediatric head imaging decision-making for advanced modalities (CT, MRI, nuclear/PET) in individuals <18 years for a wide range of childhood head disorders — e.g., headache, head/face trauma, seizures/epilepsy, macrocephaly/hydrocephalus, craniosynostosis, Chiari/skull base malformations, pediatric stroke, pituitary dysfunction, and many other pediatric-specific indications. Major limitations/requirements: advanced imaging generally requires a prior pertinent clinical evaluation (detailed neurologic exam when appropriate), basic imaging/labs as indicated, is not supported for asymptomatic screening, repeat studies need evidence of progression or documented impact on management, and atypical or unlisted presentations require individual physician review.
