Pediatric Neck Imaging Guidelines

This policy covers advanced pediatric neck imaging—including MRI (orbit/face/neck and TMJ), CT/CTA (maxillofacial and neck), MRA, CT guidance and related procedures (specified CPT codes)—for evaluation of common pediatric neck symptoms and symptom complexes in patients under 18. Major limitations: atypical presentations require physician review and patients ≥18 follow adult guidelines; a recent face-to-face evaluation (within 60 days) is generally required, gadolinium use should be limited to situations where contrast provides necessary additional information, anesthesia is usually required for most infants and children <7 (and should be minimized), and screening or repeat studies are not supported unless they will change management.
