This policy covers neck imaging services — including ultrasound, contrast-enhanced CT (generally CPT 70491), and MRI — for evaluation of a wide range of neck complaints and conditions such as cerebrovascular/carotid disease, dysphagia and esophageal disorders, cervical lymphadenopathy and neck masses, head and neck malignancies, thyroid/parathyroid and salivary gland disorders, recurrent laryngeal palsy, tracheal/bronchial problems, torticollis, and related indications. Advanced imaging is limited to patients with a recent (≤60 days) clinical evaluation including relevant history/physical, appropriate laboratory studies, and prior non‑advanced imaging as indicated (physician review required for atypical presentations); MRI is usually reserved when CT or ultrasound suggests specific findings and contrast use has important caveats (e.g., interference with radioactive iodine studies).
