Addendum to eviCore Guidelines

This addendum updates eviCore’s guidelines to incorporate the AMA 2020 CPT/HCPCS code changes across multiple programs—notably radiology (SPECT/PET tumor and myocardial imaging), vascular duplex for pre‑dialysis access mapping, interventional pain (sacroiliac injections and RF ablation), spine annular closure device implantation, cardiac device (substernal ICD leads), and pelvic floor biofeedback. It applies to patients undergoing those diagnostic and interventional services (e.g., tumor/inflammation localization, myocardial perfusion/metabolic PET, hemodialysis vessel assessment, sacroiliac joint procedures, lumbar annular defect repair, ICD implantation, and pelvic biofeedback) and imposes key requirements and limits including an effective date of 1/1/2020 for added codes, site/timing and imaging‑guidance specifics (single‑area vs multi‑area, single‑day vs multi‑day, concurrent CT when required), and deletion/cross‑mapping of numerous CPT codes that are no longer billable or must be reported under replacement codes.
