This policy covers implantation and evaluation of cardiac rhythm implantable devices — including ICDs, CRT‑D/CRT‑P, permanent and leadless pacemakers — for patients with arrhythmic disorders (e.g., long QT syndrome, non‑sustained or incessant VT), heart failure/structural heart disease requiring resynchronization, and related indications as defined in the CRID guidance. Major limitations/requirements include use of the Clinical Decision Support tool with physician review for atypical presentations, specified non‑indications that exclude certain scenarios, and prior optimization of medical therapy (generally three months of guideline‑directed heart‑failure drugs) before device consideration, with payer/Medicare policies potentially taking precedence.
