Cardiac Implantable Devices (CID) Guidelines

This policy covers clinical indications for cardiac implantable devices, including pacemakers (including leadless), implantable cardioverter‑defibrillators (ICDs), cardiac resynchronization therapy devices (CRT‑P and CRT‑D and wireless CRT), and wireless pulmonary artery pressure sensors for conditions such as heart failure (NYHA I–IV), arrhythmias, hypertrophic cardiomyopathy, and abnormal blood pressure/exercise responses. It details definite and reasonable indications and non‑indications, requires prerequisites such as three months of maximally tolerated guideline‑directed medical therapy for heart failure, and mandates physician review for atypical presentations while noting the guidelines are proprietary, subject to annual update, and intended to support—not replace—clinical judgment.
