This policy covers oncology imaging as a diagnostic‑strategies clinical decision support tool for selecting advanced and routine imaging across a wide range of cancers (e.g., CNS tumors, head and neck, lung, breast, colorectal, thyroid, sarcomas, pancreatic, renal, and many others). Major limitations/requirements include physician review for atypical presentations, age‑based imaging rules applied at initial diagnosis, a recent clinical evaluation within 60 days before advanced imaging, single approval for initial staging studies, and restrictions/precautions on contrast use (iodinated contrast allergy/premedication, eGFR <30 and GBCA limits, discouragement of repetitive GBCA), with advanced imaging generally not indicated for patients declining standard oncologic therapy.
