Peripheral Vascular Disease (PVD) Imaging Guidelines

This policy covers imaging evaluation of peripheral vascular disease and related vascular disorders — including noninvasive tests (ABI, toe‑brachial index), duplex ultrasound, nuclear medicine and other advanced vascular imaging — for cerebrovascular/carotid disease, upper and lower extremity PVD, pulmonary arterial hypertension, aortic and renal vascular disorders, hemodialysis access, AVMs, PAD/DVT/AAA and claudication. Coverage requires a recent clinical evaluation and appropriate non‑advanced testing (e.g., ABIs within 60 days) before advanced imaging, limits routine or surveillance imaging to documented need with symptom‑driven follow‑up, cautions that simultaneous arterial/venous studies are uncommon, and includes specific CPT coding and billing restrictions.
