This policy covers cardiac imaging services—including echocardiography, nuclear cardiac imaging, cardiac CT/CTA/CAC, cardiac MRI, cardiac PET, and diagnostic heart catheterization—used to evaluate chest pain, coronary artery disease, congestive heart failure, cardiac trauma, arrhythmias, and other common cardiac symptom complexes. Major requirements/limitations: a recent clinical evaluation (within 60 days) and relevant prior tests (e.g., ECG, chest x‑ray, prior stress tests or ECHO) are required; imaging must be expected to affect management, atypical presentations require physician review, exercise treadmill testing without imaging requires an interpretable ECG and adequate exercise capacity (≥4 METs), and advanced imaging is generally unnecessary if invasive angiography is already planned.
