Cardiac Rhythm Implantable Device (CRID) Guidelines

This policy covers cardiac rhythm implantable devices—including implantable cardioverter-defibrillators (ICD), cardiac resynchronization therapy devices (CRT‑D and CRT‑P), and permanent pacemakers—for evaluation and authorization of implantation. It applies to patients with ventricular arrhythmias (sustained/incessant VT, NSVT), long QT syndrome, structural heart disease and heart failure (including non‑compaction cardiomyopathy) with indications classified as definite or reasonable; major requirements/limitations include a 3‑month trial of guideline-directed optimal medical therapy (maximally tolerated beta‑blocker, ACEi/ARB and diuretic) prior to consideration, specified non‑indications, clinician review for atypical presentations, and payer/Medicare-specific coverage rules that may supersede these guidelines.
