Peripheral Nerve Disorders (PND) Imaging Guidelines

This policy covers imaging evaluation of peripheral nerve disorders — including MRI, ultrasound, CT, nuclear and newer imaging techniques — for conditions such as focal and polyneuropathies, brachial and lumbosacral plexus disorders, ALS, peripheral nerve sheath tumors, carpal tunnel and ulnar neuropathies, and related muscle disorders. Advanced imaging generally requires a current clinical evaluation within 60 days (telephone/e‑contact may substitute), MRI is preferred over CT, EMG/NCV is the initial test for carpal tunnel and ulnar neuropathy, and many imaging studies are limited to pre‑operative, severe, or tumor‑suspected indications while atypical presentations require physician review.
