Evicore's PVD Imaging Guidelines cover medically necessary vascular imaging (ABI/TBI, duplex US, CTA/MRA/CT, TEE, and select nuclear studies) for PAD, cerebrovascular disease, aortic aneurysms/dissections, venous disorders and post‑procedural surveillance, and explicitly exclude routine asymptomatic screening, obsolete nuclear/flow studies, experimental plaque morphology codes, and unjustified repeat studies. Approval requires a recent clinical evaluation (within 60 days) with focused vascular history/physical, documentation of prior tests (e.g., ABI within 60 days or exercise ABI/TBI when indicated), relevant labs (ESR/CRP for suspected vasculitis), modality‑specific clinical justification, and adherence to specified surveillance intervals.
