This policy covers imaging for peripheral vascular disease (PVD), including diagnostic and advanced imaging for peripheral artery disease, aneurysmal disease, cerebrovascular/carotid disease, upper and lower extremity vascular disease, venous disorders (DVT, venous insufficiency, IVC filters, venous stasis ulcers), AVMs, hemodialysis access, pulmonary hypertension, and aortic/renal/visceral vascular disorders. Major limitations require a current clinical evaluation within 60 days (and documentation of prior medical treatments) before advanced imaging, recent ABIs when applicable, physician review for atypical presentations, telehealth substitution only for established patients, and adherence to CPT coding rules (e.g., 93922 vs 93923 and once-per-upper/lower-extremity reporting).
