Peripheral Nerve Disorders (PND) Imaging Guidelines

This policy covers imaging of peripheral nerve disorders — including ultrasound, MRI (inc. MR neurography), CT and nuclear imaging — for evaluation of focal neuropathies (e.g., carpal tunnel, ulnar neuropathy), polyneuropathy, brachial and lumbosacral plexopathies, neuromuscular disorders, muscle disorders and peripheral nerve sheath tumors. Advanced imaging is limited to cases with a prior pertinent clinical evaluation (history, neurological exam, appropriate labs, non-advanced imaging and EMG/NCV — telehealth/other technological contact acceptable only with an established clinician), with ultrasound preferred for superficial nerves, MRI for deep or bone‑obscured nerves, CT only if MRI is contraindicated, and atypical presentations requiring physician review.
